Introduction and Objective: Ask the Experts (EXPERTS) supports U.S. individuals who screen positive for type 1 diabetes islet autoantibodies (IA) and their health care providers (HCPs). Because clinical capacity to monitor and counsel patients with early-stage type 1 diabetes remains limited, we piloted a 1-day CME-accredited mini-fellowship to build HCP confidence and capacity for early-stage type 1 diabetes follow-up. Here we describe the program and report initial evaluation findings.Methods: The in-person course combined didactics with interactive, case-based learning led by multidisciplinary faculty, plus six-monthly case-based Zoom sessions. Evaluation used the Kirkpatrick-Barr framework at pre-course (T0), end of day (T1), and 60 days (T2), assessing case-based knowledge, attitudes, self-efficacy, satisfaction/recommendation, and commitment-to-change with a brief T2 implementation snapshot.Results: Thirteen HCPs attended (4 MD, 1 DO, 2 PA-C, 1 MSN, 2 BSN, 3 RD/CDCES) from 9 states (CA, CO, IA, MI, NC, NJ, TN, TX, VA). End-of-day feedback reflected high perceived value and intent to apply course content. Mean self-efficacy increased from 4.23 (T0) to 4.95 (T1) on a 1-5 Likert scale (+0.72 points), and 100% would recommend the program to a colleague. At 60 days, respondents reported completing clinical practice action items, identifying barriers, and using EXPERTS web-based guides/tools to support decision-making. Respondents reported IA screening and follow-up reduces diabetic ketoacidosis risk and clinics should adopt an IA-based triage workflow when feasible.Conclusion: A brief, CME-accredited training with longitudinal case-based support and pragmatic evaluation may help expand national capacity for early-stage type 1 diabetes screening and follow-up.
