Introduction and Objective: Fatty liver index (FLI) is a diagnostic tool in metabolic dysfunction-associated steatotic liver disease, an end-organ complication in overweight/obesity. This post hoc analysis assessed the impact of injectable semaglutide (sema) 2.4 mg versus placebo on FLI scores in STEP 1.Methods: People with information needed to calculate FLI scores (BMI, waist circumference, triglycerides and gamma-glutamyl transferase) were included. FLI categories were defined as no fatty liver (<30), moderate risk (30-60) and high risk (>60). FLI scores, change in scores by category and proportion of people in each category at baseline and at weeks 20 and 68 were summarized descriptively.Results: Mean (standard deviation [SD]) change in FLI score from baseline to week 68 was −29.13 (24.21) and −6.95 (13.17) in the sema (n=1180) and placebo (n=547) arms, respectively (Fig). From baseline to week 68, mean (SD) change in FLI score in the <30, 30-60 and 60+ FLI categories was −17.74 (6.42), −31.87 (17.17) and −28.92 (24.88), respectively, in the sema arm and −0.63 (NA), −7.27 (18.69) and −6.92 (12.50), respectively, in the placebo arm. A larger increase in the proportion of people in the <30 FLI category was seen from baseline to week 68 with sema versus placebo (Fig).Conclusion: People with overweight/obesity treated with sema 2.4 mg had a reduction in FLI scores and improvements in disease progression.
