1150-OR: Multiorgan Fibroinflammation and Metabolic Dysfunction–Associated Steatohepatitis Are More Prevalent in Individuals Living with Type 2 Diabetes than without Type 2 Diabetes



Introduction and Objective: We investigated the prevalence of organ impairment in people with T2D in primary care.Methods: People living with T2D and no prior heart disease were recruited from 5 sites and underwent standardised MRI (CoverScan) to measure fat (visceral, subcutaneous, epicardial, pericardial, hepatic, pancreatic), inflammation (hepatic, pancreatic, renal), muscle volume (myocardial, skeletal) and cardiac function. Comparisons were made to matched controls without T2D based on age, sex, ethnicity, BMI and waist circumference (WC) from UK Biobank, and self-reported healthy volunteers with no known disease (HV). Clinical obesity defined as Lancet Commission: high WC & high BMI & any of: T2D, dyslipidaemia, hypertension.Results: 271 people living with T2D (mean age 62, 64% male, 54% white, 50% with clinical obesity), 271 matched controls without T2D from UK Biobank (similar demographics, 25% with clinical obesity), and 92 healthy volunteers, had MRI scans with blinded analysis. Fibro-inflammation in the liver and/or pancreas was more prevalent in people with T2D compared to people without T2D (49% vs 16%, vs 7% in HV, p<0.001). Steatosis was at similar levels with or without T2D (Figure).Conclusion: Approximately half of people living with T2D have liver or pancreatic fibro-inflammation, which can be diagnosed non-invasively and should be prioritised for clinical trials and treatment.
