1341-OR: Freestyle Libre Use Associated with Faster Time to Glycemic Target in People with Diabetes Not Treated with Insulin



Introduction and Objective: Timely glycemic control reduces the risk of diabetes complications. This study evaluated whether initiating FreeStyle Libre (FSL) was associated with faster achievement of A1C targets (6.5%, 7%, and 8%) than no continuous glucose monitoring (CGM) in non-insulin treated adults with type 2 diabetes (NIT2D).Methods: Using Truveta electronic health record data, adults (≥ 18 years) with NIT2D who were prescribed FSL were propensity score-matched 1:4 to non-CGM users. Baseline demographics, comorbidities, payer type, and medications were balanced (all standardized mean differences <0.1). Time to A1c targets over 12 months was assessed using Kaplan-Meier (KM) curves, log-rank tests, and doubly robust Cox Proportional Hazards (PH) model. Subgroup analysis was stratified by age (<65 vs. ≥65 years).Results: Among adults <65 years, FSL use was associated with faster achievement of A1C <6.5% (HR: 1.15, 95% CI: 1.03-1.28), <7% (HR: 1.16, 1.07-1.27), and <8% (HR: 1.19, 1.11-1.27). In adults ≥65 years, differences were significant for <8% (HR: 1.19, 1.06-1.33), but not for <6.5% and <7%.Conclusion: FSL use was associated with faster A1C target achievement overall and among adults <65 years. In adults ≥65 years, faster target achievement was seen only for A1C <8%, likely aligning with glycemic goals in older adults.
