1259-OR: Precise Carbohydrate Counting vs. Estimated Counting in Adults with Type 1 Diabetes Using mylife YpsoPump Advanced Hybrid Closed-Loop System: A Pilot Project Comparing Glycaemic Outcomes



Introduction and Objective: Carbohydrate counting is perceived as one of the most burdensome tasks in Type 1 Diabetes (T1D) but meta-analyses have shown it is an effective method in reducing HbA1c. We compared glycemic control in adults with T1D using mylife YpsoPump with advanced hybrid closed loop (HCL) system (mylife CamAPS FX app) among those practicing precise carbohydrate (PC) and estimated carbohydrate (EC) counting.Methods: This prospective pilot included 34 adults (≥19 years) with T1D on mylife YpsoPump HCL, assigned to PC (n=17) or EC (n=17) in a University Hospital, United Kingdom. In the EC group, carbohydrate counting was estimated using YpsoPump meal-size options (snack, small, medium, large), supported by dietitian with portion-size photographs. Data was analysed with paired sample T-test (SPSS) with outcomes assessed at 6 months for time in range (TIR) as the primary outcome; HbA1c and other glycemic metrics as secondary outcomes.Results: Baseline HbA1c, glucose management indicator, BMI, and social deprivation score were similar between both groups. However, PC group had a younger cohort (42 vs 54years; p=0.016) and shorter diabetes duration of T1D (22 vs 35 years; p=0.02). At 6 months, all participants showed significant improvements in TIR (67.8±10.9%; p<0.001) and HbA1c (52.97± 6.2 mmol/mol; p<0.001). However, there were no statistically significant differences between EC and PC groups for mean change in TIR (16.2% vs 17.5%; p=0.83) or HbA1c (8.1 vs 9.2 mmol/mol; p=0.67). Changes in time above range (p=0.3) and time below range (p=0.1) were not statistically significant.Conclusion: Adults using mylife YpsoPump advanced HCL system with estimated carbohydrate counting based on meal size can achieve international glycemic targets, providing a practical alternative for individuals who find precise carbohydrate counting challenging.
