Introduction and Objective: Early identification of multi-system involvement in pre-diabetic and diabetic states remains challenging. This study investigated the timing and patterns of concurrent hepatic, bone, and metabolic derangements across different age groups and glycemic states.Methods: Cross-sectional analysis of 591 participants stratified by age (19-40, 41-60, >60 years) and glycemic status (non-diabetic, pre-diabetic, diabetic). Comprehensive assessment included HbA1c, hepatic fibroscan (LSM & CAP), BMD, thyroid function, and metabolic parameters. Age-specific patterns were analyzed using multivariate regression.Results: In pre-diabetics, 37% showed elevated CAP scores (>250 dB/m) despite normal HbA1c, with age-dependent progression (28% in 19-40 years vs 62% in >60 years, p<0.001). Early diabetics (<1 year) demonstrated 42% concurrent hepatic-bone involvement. Novel thyroid-hepatic correlations emerged, with both low TSH (<0.4) and high TSH (>4.5) showing significant organ involvement (42% and 58% respectively, p<0.01). Age-stratified analysis revealed progressive multi-system involvement: young adults (28% hepatic, 22% bone), middle-aged (45% hepatic, 38% bone), and elderly (62% hepatic, 58% bone).Conclusion: Multi-system involvement occurs earlier than previously recognized, with distinct age-specific patterns. Findings suggest the need for age-stratified screening protocols and early intervention strategies, particularly in pre-diabetic states.
