Early Onset May Lead to Faster Cognitive Decline

Entering menopause at an earlier age may be associated with a faster rate of cognitive decline.

In a study published in JAMA Network Open on August 25, researchers found a significant association between early menopause and less favorable functional and structural neurological outcomes.

The findings suggest that the association between menopause timing and brain health may persist for decades after the menopause transition.

The researchers noted that understanding this lasting association between the onset of menopause and cognitive decline offers a vital window of opportunity for targeted prevention and clinical interventions.

Individuals were included in the study if the age of their first period (menarche) and the age at which they entered menopause were recorded. They were also included in the analysis based on data availability, including:

  • longitudinal cognitive trajectories (2,603 people)
  • neuropathologic outcomes at autopsy (1,253 people)
  • longitudinal MRI scans (774 people)

The researchers found that earlier menopause was associated with earlier clinical Alzheimer’s disease onset, less favorable cognitive aging trajectories, and faster late-life brain atrophy.

These findings suggest that the age at which ovarian hormone loss occurs remains associated with structural brain aging, even decades after menopause.

“This study reinforces that the connection between menopause and cognitive decline needs to be addressed directly with patients, including education about what to expect and honest conversations about risk, particularly for women with earlier or surgical menopause,” Sherry Ross, MD, board certified OB/GYN and Women’s Health Expert at Providence Saint John’s Health Center in Santa Monica, CA, told Healthline. Earlier menopause was associated with a faster accumulation of white matter hyperintensities, which are areas of damaged brain tissue visible on an MRI. This association was found in women who experienced spontaneous menopause, but not those who experienced surgical menopause.

The study found that those whose menopausal transition occurred five years earlier had 15% higher WMHV.

The average age of Alzheimer’s disease diagnosis for the study cohort was 88.4, which translates to approximately 1.8 fewer years free of the condition for a woman whose menopause began 10 years earlier. This corresponds to a 2% faster onset of Alzheimer’s disease per decade of earlier menopause.

Ross noted that estrogen has powerful and protective effects on brain function. It supports the way brain cells and nerves work together and helps manage energy use and inflammation in brain tissue.

Estrogen plays a much bigger role in the body than most women realize, well beyond reproduction, she told Healthline.

“As more research continues to explore what happens when estrogen is lost, women will have a better understanding of why so much changes throughout the body during this transition, not just in the brain, but in areas we are only beginning to study,” said Ross.

When menopause occurs early, these protective effects are lost sooner, according to Ross.

“When you enter menopause matters when it comes to brain health,” she said. “How menopause happens matters too.”

In the study, women who experienced surgical menopause following ovary removal, which causes a sudden loss of estrogen, had faster memory decline and an earlier Alzheimer’s disease diagnosis. In contrast, women who experienced menopause naturally, with a more gradual loss of estrogen, appeared to have a slower buildup of structural brain changes over time.

Ross told Healthline that, “Women going into early menopause, surgical menopause, or who have cardiovascular risk should be identified so their cognitive functioning can be followed over time as it relates to memory decline and Alzheimer’s risk.”
