Introduction and Objective: Gestational diabetes mellitus (GDM) affects ~12% of pregnancies and is associated with maternal and offspring perinatal complications and long-term metabolic risk. We assessed GDM-related knowledge gaps, perceptions, and support needs among women aged 20-49 years to inform seamless support across preconception, pregnancy, and postpartum periods.Methods: A nationwide cross-sectional web survey included women aged 20-49 years. From a survey panel, 4,000 women were randomly selected; those with chronic diseases or who were healthcare professionals were excluded. The survey assessed GDM knowledge, health information sources and reliability concerns, and preferred support modalities; women with a history of GDM (GDM group) reported education, care experiences, and self-management challenges. Ethical approval and consent were obtained.Results: After exclusions, 3,768 women were analyzed, including 1,874 with no childbirth history, 1,779 with childbirth history without GDM, and 115 in the GDM group. (1) Knowledge gaps: Basic GDM knowledge was low among women without GDM (4.1-15.2%) and remained limited in the GDM group (40.9-58.3%). (2) Information uncertainty: Non-medical sources were commonly used, and over 70% reported concerns about information accuracy. (3) Care gaps: Among the GDM group, 29.6% received no antenatal guidance, 57.4% no intrapartum follow-up, and 51.3% no postpartum care; 37.4% had no check-up within 2-3 years postpartum, mainly due to lack of awareness. (4) Digital support needs: Need for self-management tools was high (56.4%, 58.2%, and 77.4%), with preferences for apps, digital media, and online communities.Conclusion: Strong demand for digital health-enabled, multidisciplinary team-based care highlights the need for accessible support integrating healthcare professionals across preconception, pregnancy, and postpartum to prevent perinatal complications and strengthen long-term diabetes prevention.
