Introduction and Objective: The aim of this study was to analyze the effectiveness of a tubeless AID system on glucose metrics in patients with type 1 diabetes (T1D).Methods: We collected glucose data (TIR, TBR, TAR, CV GMI and GRI) from 94 T1D patients (age 37±13 years; M/F = 35/59; diabetes duration 19±13 years; HbA1c 7±1%; 14% with retinopathy, 4% with cardiovascular disease, 6% with nephropathy, 5% with neuropathy and 1% with diabetic foot complications) at 15 (T1), 30 (T2), 60 (T3), 90 (T4) and 180 (T5) days after initiation (T0) of Omnipod 5. Previous therapy was Omnipod Dash (69%), another AID (6%) or MDI.Results: Use of Omnipod 5 was associated with a significant improvement in all glucose metrics, in particular GRI (41.3% T0 vs 28.2% T1, 29.5% T2, 28.6% T3, 28.9% T4, 29.1% T5; p<0.001). In addition, at T5, GRI showed a positive correlation with TAR, CV and GMI and a negative correlation with TIR (all p<0.001) but it didn’t correlate with TBR.Conclusion: Omnipod 5 rapidly and significantly improved all glucose metrics, especially GRI. After 180 days of its use, GRI correlated with all the glucose metrics with the exception of TBR, probably because of the low incidence of hypoglycemia in the population evaluated (mean TBR 1.9%). These results support the usefulness of GRI in assessing glycemic control and treatment efficacy.
