Introduction and Objective: This pilot study aimed to examine feasibility and acceptability of a telehealth DSMES intervention, Deaf Diabetes Can Together (DEDICATE), to support Deaf and hard of hearing (DHH) adults with type 1 and type 2 diabetes.Methods: Diabetes DHH Participants completed a 10- to 12-week multicomponent telehealth intervention in ASL consisting of DSMES ASL videos, small group discussion, and weekly goal setting delivered by a DHH community health worker and ASL-fluent clinician. A companion website stored videos for repeated viewing. Data collection included A1C, and participant reported outcomes at baseline, post intervention (with interviews), and 24-week follow-up.Results: Twenty-five DHH participants (52% female, age 53.2 ± 9.9 years, diabetes duration 14.7 ± 11.1 years; 32% non-White/Hispanic, baseline A1C (7.0 ± 1.1 %) enrolled in the study. DEDICATE demonstrated feasibility as evidenced by 23 (92%) attending at least 70% of the sessions and survey completion of 96% at post and 92% at follow-up. Post-intervention, 4 (16%) had a clinically significant decrease in A1c (≥0.5% decrease), 17 (68.0%) maintained A1C (≤0.5%), 2 (8%) increased ≥0.5%, 2 (8%) missing. Follow-up, 6 decreased A1C, 12 maintained, 3 increased, 4 missing. Diabetes self-care inventory, diabetes self-efficacy, and diabetes distress all improved post and follow-up (partial η² from 0.026 to 0.134). Out of 206 total weekly exercise, food, and medication goals set, 94.6% were met or partially met. Participant interviews highlighted accessibility, meeting the linguistic and cultural needs of the DHH population. Participants noted difficulty with the companion website and only rewatched videos if they missed group sessionsConclusion: Findings show evidence of feasibility and acceptability of DEDICATE in the historically under-represented DHH population. A larger randomized control trial powered to detect significant improvements in A1C, and patient reported outcomes is ongoing.
