Introduction and Objective: Implementing lifestyle interventions guided by continuous glucose monitor (CGM) data can potentially prevent progression and reduce risk of prediabetes and type 2 diabetes (T2D). Maintaining healthy behaviors is particularly difficult in high-stress work environments like health care workers (HCWs). Sutter Health created a Metabolic Wellness Program (MWP) to educate on interpreting CGM data to help make personalized and sustainable lifestyle changes. We sought to understand the experience and impact of MWP for HCWs in-depth.Methods: At least three months after participating in MWP, two researchers conducted semi-structured interviews that were audio-recorded and transcribed. De-identified transcripts were uploaded into Databricks, and we used Open AI’s GPT OSS 120b to generate initial themes and supporting quotations. Themes were validated, refined, and finalized by human qualitative experts.Results: We interviewed 29 participants (7 metabolically healthy, 11 with pre-diabetes, 11 with T2D). Many participants mentioned their main motivations for joining MWP were family history and/or currently had prediabetes/T2D. Many said that they felt these sessions were better than primary care as they received “more in-depth” and specific information than general advice of “manage weight and exercise.” Participants reported mixed feelings about CGMs, with some expressing benefit from directly seeing impact of food and exercise while others noted additional “anxiety” from “spikes” and “alarms.” Sustained behavior changes months after MWP were also mixed, with some making “lasting changes” through increased exercise after meals and “eating carbs last” while others missed “accountability” from MWP or not having additional CGMs.Conclusion: The findings suggest that MWP may help to improve metabolic health and provide some sustained behavior change to potentially decrease the risk of diabetes and pre-diabetes in HCWs.
