1350-P: Community-Supported Initiative for Diabetes Screening and Management in a Rural Community in Bangladesh



Introduction and Objective: This study aimed to evaluate the feasibility of a community-supported initiative for diabetes screening and management in a rural community in Bangladesh.Methods: Our community-supported initiative comprised design ethnography to understand the community and identify local resources. Then, community organization was facilitated by forming a health and inclusion committee after obtaining approval from each household. All individuals over age 18 were screened. At least 8 hours of fasting glucose measurements were used for screening: above 7.00 mmol/L (126 mg/dl) for diabetes and 5.6 mmol/L (100 mg/dl) to 6.9 mmol/L (125 mg/dl) for prediabetes. Fasting and 2-hour post-prandial glucose of the diabetic patients were measured every 15 days between June 2022 and May 2024. Lifestyle intervention and medications were given utilizing local resources and community organization.Results: 366 individuals were screened. 34 (9.29 %) screened positive for diabetes, and 47 (12.84%) were positive for pre-diabetes. The median age, weight, and fasting glucose of the diabetic patients were 52 years, 55 kg, and 10.4 mmol/L (187 mg/dl), respectively, and of the pre-diabetic patients, were 40, 54 kg, and 6 mmol/L (108 mg/dl), respectively. Of the diabetic patients, recurrent infection, delayed wound healing, or blurry vision was reported by 3 (8.82%), 6 (17.65%), and 23 (67.65%) individuals, and of the prediabetic patients, 8 (17.02%), 9 (21.28%), and 17 (36.17%) individuals. Of the diabetic patients, 70.97% were educated in 8th grade or below, 74.9% were homemakers, 12.9% were disabled, and median monthly expenditure (proxy of income) was17000 taka (approximately 170 USD)Conclusion: The project demonstrates the feasibility of the initiative in screening for diabetes in resource-limited settings. Further analyses of prospectively monitored glycemic indices with medication and lifestyle management of identified diabetic patients are ongoing.
